Asking for any and all records related to the police call at my residence on Friday the 17th at or about 1:55 PM, including, but not limited to statements made to the police, medical records that the police are in possession of for both parties, copies of charges made or filed, all photographs taken by the police, and any other information that the police are in possession of regarding the incident. 

Also, that I believe that the copying fee should be waived because it is in the interest of public safety and that I will primarily use the records to assist in making the emergency order of protection against Austin permanent. 
I plan to submit this as an exhibit in the protective order court date against Austin to show the judge what happened and what I looked like when Austin was finished with me.
I also plan to submit my medical bills and ask the court to award damages to me in that amount.
